Under the direct supervision of the Director of Housing and Client Prosperity, the Housing Navigator is primarily responsible for assisting WEAVE’s clients with achieving their housing goals. The Housing Navigator will assess the housing needs of clients, provide support and advocacy with securing and maintaining safe and stable housing, and works closely with Sacramento County’s Coordinated Entry System, the Sacramento Housing and Redevelopment Agency, and housing providers in the community.
- Supports clients with establishing housing goals by conducting housing assessments to determine clients’ housing needs, affordability, and housing preferences.
- Creates and/or updates clients’ HMIS profiles to explore housing opportunities and conducts referrals through the traditional Coordinated Entry System.
- Conducts and/or updates clients’ Vulnerability Index Assessment (VI-SPDAT), to ensure housing opportunities and conducts referrals available through the traditional and/or the Survivor Coordinated Entry System.
- Supports with locating and building housing partnerships based upon clients’ housing preferences, family size, and income through mobile advocacy.
- Supports clients with applying to housing programs with the local housing authority or housing programs in the community.
- Provides housing case management and advocacy by assisting clients with obtaining housing, including supporting clients with housing search, rental applications, applying to waiting lists, accompaniment to signing the lease, accompaniment to property tours, collecting required documentation for housing applications, and other housing related support as needed.
- Supports clients with the moving process, including scheduling movers, coordinating household establishments items, and providing resources and referrals.
- Conducts WEAVE’s Housing Orientation to provide an overview of the Housing First voluntary services available.
- Conducts economic stability assessments to evaluate clients’ financial wellness and requests flexible financial assistance from WEAVE and other community partners as appropriate.
- Provides support, advocacy, and referrals to lower clients’ barriers and increase their housing opportunities.
- Conducts the Ready to Rent workshops one-on-one with clients or in a group setting and adapts the delivery of the curriculum according to the clients’ specific needs.
- Ensures clients are provided with internal referrals to WEAVE Services, including individual and/or group counseling, and legal assistance as well as services and resources in the community based on individual client needs such as housing, public benefits, employment, legal assistance, health services, pet supportive services, and others.
- Maintains high visibility and availability throughout the respective housing program, initiates contact with residents, and provides a welcoming and nonjudgmental environment.
- Provides emergency transportation, food, and clothing.
- Co-facilitates house meetings, support groups, and educational groups / workshops.
- Maintains accurate client records and statistics in a timely manner.
- Maintains close communication with other staff.
- Processes authorized flexible financial assistance requests with appropriate back-up documentation to the Finance Department and serving as the liaison between the Finance Department and either the client, the Advocate, or a partner to ensure payment is made accurately and timely.
- Maintains the Flexible Financial Assistance Funds Tracker and ETO data for flexible financial assistance accurately and timely.
- Assists with obtaining required documentation from property management companies, landlords, vendors, subcontractors, etc., such as W-9s, lease agreements, invoices, etc.
- Supports with maintaining a list of external partners available to assist with client prosperity, including property managers, employers and staffing agencies, partners available to provide client workshops or resources related to employment, finances, and housing.
- Attends and participates in agency meetings, staff meetings, and in-service trainings.
- Completes WEAVE’s classroom training (69 hours) and maintains certification.
- Develops positive working relationships with community partners and other local organizations and agencies.
- Maintains accurate and timely timesheets.
- Maintains a working knowledge of domestic violence, sexual assault, elder abuse, child abuse laws and reporting responsibilities and files abuse reports as required.
- Be familiar with legal, medical, housing, educational, and social service agencies and resources in the community which may be of use to residents.
- Other duties as assigned.
ADA Job Characteristics:
Work environment is primarily in an emergency shelter/ 24-7 facility setting. Local travel (driving) is required to transport clients and attend off-site meetings. Lift and carry up to 25 pounds up to waist length, extend both arms above the head and/or reach below the waist, stoop, squat, crawl, bend the back to open lower-level filing drawers, or to retrieve stored items, climb on stools, steps and/or ladders to dust, sweep and/or mop to clean. Sit and/or stand frequently. Use fine manipulation (hands and wrist) to operate a keyboard, take notes, complete and file reports, file and copy documents. Corrected vision to normal range. Speak in normal vocal tone and range and communicate clearly and concisely to and with others. Apply normal reasoning and detail as required in the accomplishment of job duties.
- Combination of formal education and experience related to the position description and ability to successfully fulfill position responsibilities.
- Ability to relate in a supportive, non-judgmental manner and maintain a client-centered approach within the parameters of agency policies and procedures.
- Understanding of the impact of trauma and ability to provide trauma-informed support.
- Experience working with survivors of domestic violence preferred.
- Crisis intervention, problem-solving and conflict resolution skills.
- Excellent oral and written communication skills.
- Ability to work both independently and as a team leader.
- Valid California driver license, reliable transportation, and proof of vehicle insurance.
- Ability to work evenings, weekends, and holidays, as needed.
